New Patient Registration
To register at the Elms you need to bring 1 form of ID (Passport/Birth Certificate or current utility bill) you will need to complete a new patient registration questionnaire.
Please print off and complete the attached New Patient Questionnaire form FOR AGES 16 AND OVER ONLY and bring it with you to the practice (during normal opening hours). You will complete your registration at the practice and initially be asked to make an appointment with our Healthcare Support worker. It is important you tell us and/or the Healthcare Support worker about any existing long term conditions you may have or any regular/long term medications you may be taking.
If you require to register a child under the age of 16 please contact the practice directly/in person to obtain a child registration pack.
