Patient Participation Group (PPG)
Last Updated: Monday, 09 December 2024The Elms Medical Centre and their Patient Participation group work together to support change and improve how we deliver health care to our patient population. We discuss challenges faced by the practice and wider health system, and agree objectives and projects that aim to improve health outcomes and reduce inequalities in access to health care.
Health Promotion and Prevention was at the forefront of our agreed objectives. We agreed that whilst there were lots of possible projects, they must be achievable within realistic timescales. This will be a rolling process and constantly reviewed at our meetings.
The PPG meet every 4months with continual contact throughout the year in focus groups and informal catch ups. If you would like to be part of the PPG or a specific project, please email us: GP.N82070@nhs.net
Our current improvement areas are:
- Digital Services: Enable patients to utilise digital resources and manage their health through the NHS app, including ordering prescriptions, managing appointments and referrals, getting advice and preparing them for the national roll out of the NHS passport. Improve digital inclusion whilst being mindful, not exclude those who do not want to engage with digital services.
- Health Promotion & Prevention: Raise awareness around current health issues, how to access services and promote the right person at the right time philosophy. Improve communication amongst the practice and the patient population, improving access and reducing inequalities and ultimately improve health outcomes.
-
Sustainability and Greener healthcare: Encompassing all aspects of sustainability including how we access the practice, processes within practice, reducing our carbon footprint and greener medicines.
