Chase your Referral?

If your referral has been sent through e-Referral service, you will receive a "Booking Letter" either during your appointment or sent to your home address, all important information on this letter will be highlighted. 

*Please be sure we have all your up to date contact details including your living address* 

On your e-Referral Booking Letter you will see the following information on it:

On this letter it will provide you with information of what to do if you have not been contacted by the date given on the letter. 

If you feel the problem you have been referred into Secondary Care for, is getting worse.  Please contact The Elms Medical Centre during our triaging time and ask a member of our helpful Reception team for a triage call with our triaging GP.

 

Chase your Hospital Appointment?

If you are currently under a specialist team/department or consultant, you may find on a letter you have received from them, a Department/Secretary Telephone Number. 

Alternatively, calling the Hospital's Switchboard and asking for your call to be transferred through to the Specialist Team/Department/Secretary.

If you are unable to speak to a member of the team, but are able to leave a voice message, provide your name, date of birth, hospital or NHS number (if you have it) and a contact number so they can call you back. 

If you have been referred and are awaiting a first appointment within the service, it is best to call the Patient Access Team (number is located on the letter you will have received either in Practice or by post).

 

Where We can help?

We understand some patient may find it difficult to chase their referrals/hospital appointments or letter due to many different reasons.  The Clinical Team will review your request and forward this over to the Patient Chasing Team and request they assist the patient.

This can take us a little longer as we fit this in-between other daily duties but we will always try to help as best & as quickly as we can.
