Self-Referrals Information Page
Over the past few years the way referrals can be made have changed a lot. There are plenty of services now available were it is no longer required for you to be directly referred by you GP.
NHS Merseycare, Talking Therapies:
We provide psychological treatments, sometimes called talking therapy, to help people with common mental health problems such as feeling stressed, feeling low in mood (depressed) or very nervous (anxiety).
This free and confidential talking therapy service is available to adults aged 16 or over who are registered with a Liverpool GP.

If you or a loved one are having a mental health crisis, you can call a local NHS mental health helpline for 24-hour advice and support:

You can call for yourself, your child, your parent or someone you care for.
If someone's life is at risk or they cannot be kept safe, call 999 or go to A&E.
Podiatry Service:
If you live in Liverpool, the Podiatry service is provided from community sites in addition to patients own homes, residential and care homes. For an assessment appointment, please download the application form, complete it and email podiatry.referral@merseycare.nhs.uk
Alternatively, you can complete a paper referral at the Surgery and return for sending.

Trans Support Service (TSS) self referral:
The service is only available to people who are resident in Liverpool, Sefton and Knowsley.

Alternatively, if you would prefer to complete the form by telephone, please call the service on 0151 247 6555 and they will arrange a time for a Trans Support Service therapist to call you.
Liverpool CAMHS:
